Forms Required Before Your Child’s First Day

In accordance with California state law, we must have the following forms completed for each child before he/she can attend Children’s Corner:

• Physicians Report, including documentation of immunizations, signed by your child’s physician

• Enrollment Contract and Membership Agreement

• Parent’s Health Report

• Membership Information

• Photo Authorization

• Field Trip Permission

• Parent Handbook Acknowledgement

• Medication Policy Agreement

• Parents Rights

• Personal Rights

• Parent Policy Agreement
