In accordance with California state law, we must have the following forms completed for each child before he/she can attend Children’s Corner:

- Physicians Report, including documentation of immunizations, signed by your child’s physician
- Enrollment Contract and Membership Agreement
- Parent’s Health Report
- Membership Information
